Pharmacological Action: Clopidogrel bisulfate is a platelet aggregation inhibitor that blocks platelet activation and aggregation by irreversibly inhibiting the ADP receptor (P2Y12), thereby preventing atherothrombotic events.
Indications: Used in the prevention of atherothrombotic events in patients with myocardial infarction, ischemic stroke, peripheral arterial disease, and acute coronary syndrome, often in combination with aspirin.
Pharmacokinetics: The drug is metabolized by the liver to its active thiol derivative before onset of action, with a half-life of approximately 8 hours. Following oral administration, approximately 50% is excreted in the urine and approximately 46% in the feces within 5 days.
Dosage and Administration: Generally, adults should take 75 mg once daily, with or without food. In patients with acute coronary syndromes, unstable angina, or non-ST-segment elevation myocardial infarction, initial treatment should include a 300mg loading dose, followed by a daily maintenance dose of 75mg. For patients with ST-segment elevation acute myocardial infarction, the daily dose is 75mg, with the 300mg loading dose advisable depending on the patient's condition.
Adverse Reactions: Major adverse reactions include an increased risk of bleeding, such as gastrointestinal bleeding and intracranial hemorrhage. Gastrointestinal discomfort and rash may also occur.
